A current photo driver’s license or MVC non-driver ID, photo Welfare or Medicaid card, employment photo ID, or passport.
Children
Relatives under 18 come with an adult relative, who brings written parental consent.
What to Wear
No skin-tight, sheer or see-through clothing (leggings and rompers included), bare shoulders, midriffs or cleavage, tank or crop tops, hoodies, scrubs, khaki, orange, gray or camouflage, flip-flops or slides.
Lockers
Lockers cost $1.00, quarters only, and there is no change machine. Bring four quarters.
Monday Through Friday at Mid-State
