Because they copied the wrong half of one line. New York publishes a business address and a separate incarcerated mail box for each prison, and procurement documents carry only the business one. That is where Caller Box 400 comes from.

Wednesdays and weekends, 9:00 AM to 3:00 PM, with the latest arrival at 2:00 PM. Wednesdays are open to all, and each weekend day belongs to even or odd DINs. Weekday holiday visiting is suspended at Five Points, so a public holiday falling on a Monday or a Thursday is not a visiting day here.

A minimum of three hours, unless it starts after 12:00 PM. That is a published floor rather than a guess, and it is worth knowing before you drive, because the three hour mark is also when the visiting room starts asking people to leave if it is full.

Do I need to apply before visiting?

No. New York has no visitor application and no approved-visitor list for an ordinary visit. You need valid photo identification and the consent of the person you are visiting. Former incarcerated people, anyone on probation or parole, and DOCCS employees need written permission from the Superintendent.
